Registered Student Organizations (forms)

Toothbrushes!By registering with HSC Student Affairs, student organizations are granted use of the David L. Boren Student Union, the ability to apply for HSC Student Association funding, and the ability request approval to host an event with alcohol service. Student Organizations must register each year and must have a faculty/staff advisor.

Registration does not necessarily imply university endorsement of the purposes of the organization. Approval of student organizations and their related activities shall be based upon such considerations as performance, educational purpose and other criteria related to the goals and objectives of the organization, purposes of the activities and how they fit within the mission and scope of the University of Oklahoma Health Sciences Center. Membership in student organizations cannot be restricted by gender, race, color, religion, national origin, age disability or veteran status. Membership may be restricted by academic area, educational level (undergraduate, graduate, professional, etc.) or other academic/professional status.

How to register your organization

To register your organization, please complete the following paperwork and return to HSC Student Affairs, David L. Boren Student Union, Suite 300.

Requirements for registration

  1. Completion of HSC Online Student Organization Registration
  2. Establishing a statement of purpose for the organization
  3. Names, signatures and ID numbers of 10 current OUHSC students as founding members with designated officers (President, Vice President, Secretary/Treasurer, etc.)
  4. The name and signature of a full-time OUHSC faculty or staff member as the Advisor/Sponsor for the organization

Before approval is granted, applications will be reviewed by HSC Student Affairs and the HSC Student Association.

How to receive funding for your organization

Each fall, the HSC Student Association distributes funding to student organizations registered through HSC Student Affairs. Application for funding is due at the end of September. The funding application will be made available upon completion of an online organization registration packet.
